Reward Redemption Form

The customer confirms having received the reward or value of gift cards mentioned above. This refers to the agreement between Westburne and your business and is based on Westburne's "We Stock It" inventory initiative by the beneficiary company (less taxes, returns, and credits) as determined by Westburne. The beneficiary company is responsible for reporting the value of the rewards and (or) gift card to the tax authorities.
